Uso nocivo y potencial de abuso de la quetiapina. Reporte de caso y revisión de la evidencia

Resumen

La quetiapina es un antipsicótico de segunda generación que tiene un amplio espectro de empleo en el tratamiento de varias enfermedades psiquiátricas, incluyendo a los trastornos por consumo de sustancias. A pesar de su eficacia y usos aprobados, hay varios informes de su potencial de abuso. En este artículo se reporta el primer caso en Hispanoamérica sobre uso riesgoso de quetiapina. Además, se incluye una revisión de la literatura que toma en cuenta reportes de caso, estudios epidemiológicos y estudios en modelos animales. El uso riesgoso o patrón de búsqueda-uso compulsivo de la quetiapina se reporta en individuos vulnerables con historial de trastornos por uso de sustancias, a menudo por consumo de cocaína. Se discuten las hipótesis sobre este fenómeno. A pesar de que la evidencia más fuerte de este tema procede de estudios preclínicos y no de mejores fuentes, los informes de casos como este deben ser tomados en cuenta en la evaluación de los pacientes, en cuanto a la utilidad de esta droga y sus posibles riesgos en el tratamiento de trastornos por uso de sustancias.
